Once you have filled out the intake form, please return to the schedule a consultation page, to select the type of visit and to book a time.
Confirm your coverage
Before scheduling a tele health consultation, confirm that your insurance plan covers tele health. Generally Aetna and UHC plans only cover in home visits.
